New Leaf Physical Therapy and Wellness PLLC is an out-of-network provider and accepts payment at time of service. Cash, check, credit card, HSA (health savings account), and FSA (flexible spending account) are accepted forms of payment. I can provide you with a superbill to submit to your insurance for potential reimbursement. Clients with Medicare cannot submit for physical therapy reimbursement. Virtual coaching is not eligible for insurance reimbursement.
